Chronic Renal Failure And Erectile Dysfunction

Erectile dysfunction (ED) is commonly found in majority of patients suffering from CRF (Chronic Renal Failure). Nearly 9 percent CRF patients in the pre-dialysis stage, 70 per cent in the dialysis stage, and 43 per cent in transplant stage are reported to have been suffering from severe sexual dysfunction. Both psychological and organic factors are responsible for the development of this condition in the CRF patients.

The condition becomes even worse in uremic patients with diabetes. This may be largely attributed to the functional disruption of the endothelium-mediated and neurogenic mechanisms in such patients.

The human brain processes several sexual stimuli and transmits them in form of parasympathetic impulses to the sex organ or the penis. Consequently, the penis arteries undergo vasodilatation, the veins get compressed against the firm tunica albuginea, and the smooth layer of the organ gets relaxed. This in turn causes the blood to rush to the penis’ erectile tissue under very high pressure, thus making the organ firm and elongated. This mechanism of penis erection is however disturbed or lost in patients having CRF or ESRD (Renal disease in end-stage) as these diseases disrupt the patient’s hypothalamic-pituitary-gonadal axis, affecting the release of GnRH, a vital hormone to ensure reproductive capability.

Other factors like stress, fatigue, systemic illness, decreased plasma levels of testosterone and LH, depression, and inadequate intake of nutrients in patients exhibiting renal failure further suppress the release of GnRH leading to erectile dysfunction and hypogonadism. Besides, in nearly 78 percent patients having CRF or ESRD, the occlusive disease of the cavernosal artery is the main reason underlying erectile dysfunction. In many other cases of renal failures, factors like smoking, hypertension, and hyperlipidemia are known to cause penile atherosclerosis that eventually lead to ED.
